Case Management makes a difference

Vita Health is committed to providing members with quality health care while providing important health information and education that promotes healthy lifestyles. In support of this commitment, Vita Health has implemented a medical case management program and member-oriented education programs. Each of our Case Management programs provides direct member, parent, caretaker contact to ensure access to the necessary health care services, utilization of these services and the educational information necessary to promote and necessary to promote a healthy lifestyle.

• The Awesome Airways Program is designed to assist children, teenagers and parents to understand, control and promote a healthy lifestyle while living with asthma. This program's focus is on member and parent education and the promotion of improved compliance with asthma medications and established treatment protocols.
• The Healthy Child Program is designed to encourage access to preventive health care services. Focus is placed on promoting well child visits, scheduled immunizations and promoting primary care utilization.
