Vita Health makes it easy for you to become a member with comprehensive and affordable health coverage. We believe that everyone has the right to be healthy and have access to the services necessary to stay healthy. As a shared-cost health coverage plan, members are responsible for a portion of the monthly premium. The additional share of your premium is subsidized by the Health Care District of Palm Beach County.

Becoming a member is easy…

Step One: Please print the application, complete with your personal information and sign

Step Two: Enclose a check for your first month's premium

Step Three: Return the completed application and required documentation to:
